Way late to the party but I had the EXACT same issue.  Have changed tires myself numerous times and axle torqued to 70-80 ft. lbs.  This time the right spacer did the same thing as gbw's.  I was lucky and knowing the spacer was going to be sacrificed, mine popped free after working it back and forth with channel locks forever using generous amounts of  penetrating oil.  I will be using less torque from now on.  Now waiting on new spacer...

Wow. Sorry for your misfortune. Guess at least I'm no longer alone in having this happen.

I'm sure my problem must have been way over-tightening the axle.  I'm very careful now to not overdo it.  Had had the front wheel off several times since my original post for new tires without further issue.

Glad its out now, just in case anyone else gets the same problem, I had it years ago on another bike, but used a ratchet strap through the front wheel and across both forks, stopped them splaying.
